Jason Giambi (torn left plantar fascia) has been removed from a walking boot, but he is still out indefinitely. … Doug Mientkiewicz (right wrist) and Darrell Rasner (right index finger) will head to the Yankees’ Legends Field training facilities in Tampa, Fla., during the All-Star break to begin their rehabilitation. … Phil Hughes has thrown a bullpen session and is scheduled to continue throwing batting practice sessions at Legends Field, shooting for an early August return. … Right-hander Jeff Karstens is progressing steadily in his recovery.
